Use it or LOSE it! Visits are only valid from one payment period to the next. Unused visits DO NOT transfer to the following month!

Single Adult: $119 for 4 visits during the billing cycle

Spouse: $59 for 4 visits during the billing cycle

Family under 18: All of your children are eligible for 4 visits during the billing cycle for $49

Have 1 kid? $49 a month for them! Have 4 kids? Still only $49 a month!

This plan is a 3 month commitment, not a one-time payment.
